Premier Family Medical participates in many of the managed health care plans currently in the market. To find out if we are listed with your plan, review your benefits package or call our office at 973-403-3200. Be sure to bring your insurance card ID with you to each visit and inform us promptly if your insurance plan has changed. Our office staff will file your insurance claims if you provide us with the appropriate information.
Insurance companies usually require a co-payment at the time of service. Payment at time of service for co-pays is mandatory, and co-payments can be made in cash, check, or credit card.
We accept assignment on all Medicare patients. It is important to note that Medicare requires an annual deductible and patients are responsible for this. Also Medicare does not pay for strictly preventative services. If there is a question about what Medicare may cover, you will be asked to sign a waiver before proceeding with the test.
Please familiarize yourself with your particular health plan, and what is covered and not covered, as plans vary significantly. If you have specific questions regarding your insurance coverage, please contact member services for the plan. At each visit we will ask for a current and valid insurance card and with the first visit we ask for a copy of your driver’s license. For patient without coverage by any insurance plan (self pay), payment at time of service is expected.
